[PATCH] Correct /proc/partitions for MD

Here's a small patch to correct the output of /proc/partitions
for the MD devices. It changes it from something like
/dev/mdb (wrong) to /dev/md1 (right). This looks like its
needed for the new LVM tools to detect LVM paritions on
MD devices.
        
Patch should apply against any late 2.3.
