[patch][rfc] per-queue request free list and modular elevator

Hi,

I've put up a new version of my block patch, which adds per-queue
request free lists and modularizes the elevator and makes it selectable.
Two elevators currently exist - the one we have now (ELEVATOR_DEFAULT)
and a no-op "elevator" that always merges and never sorts.
