[PATCH] Documentation/*/00-INDEX files

Hi,

Patch to Documentation/00-INDEX files - also did networking and
filesystems subdir index files (which might even be ones I originally
started...) - other subdirs can be dealt with by their respective
maintainers or whoever is bored enough.
