would you please provide the following information:
kernel version you use
# mkreiserfs -V
raid configuration file which you used to create /dev/md0
# cat /proc/mdstat
# pvs
# lvdisplay --maps
# pvdisplay -v
# vgdisplay -v
# ll /dev/mapper
