[PATCH 32/32] ide-tape: cleanup the remaining codestyle issues

Which is, in particular:
- reorganize #defines at the top
- whitespace cleanup
- remove the "!= NULL" chuck from the pointer validity tests since the short
version "if (ptr)" is more intuitive
- fix comments
- remove unused "length" arg from idetape_create_read_buffer_cmd()
- shorten some vars in order to fit lines into 80 columns.
- include proper headers
- finally, bump driver version

thus decreasing checkpatch errors to 0.
