Hi Linus,

Three regression fixes, plus one further fix for the oom_cfqq fallback
in cfq. The congestion_wait() fallout is mostly one-liners, bringing the
wait/wakeup/set/clear bit usage in line again after the sync/async
change in 2.6.30.

I'll be away the next two weeks, but Tejun has promised to keep an eye
on important block layer bits while I'm away.
