[PATCH 0/3] sp0, ss1, and sp1 docs and minor fixes

This documents things, clarifies the code, and fixes an apparently
inconsequential bug that caused the init sp0 to be wrong.

(My earlier 32-bit bug was caused because I thought that the code
I'm fixing was correct. Whoops.)
