race condition in md (Re: First snapshot of a proposed set of fixes to make 2.0.34)

I am still awaiting the patches to the md driver which were supposed to
get rid of the locking anomaly (or did I miss them?).
If they don't happen, I propose adding the minimal patch to the two
disk-wait-queue loops which gets rid of the race conditions I am experiencing
here.
