IMHO hardware and software are not the problem.
Here the problem is the human operating the computer !
>From the ls -l you show us, I can see that there is a
frame3.html
^^
and a
frame3.hmtl
^^

No bugs except the one you spotted in your eye -> brain association (may
be corruption errors due to an overclocked organic data bus :)
