I just saw my very first instance of this problem. It occured during a
torture test where I try to tar the entire filesystem to /dev/null from a
remote NFS client:

IBM 9LZX U2 LVD SCSI disk on built-in Adaptec 7890/1 controller
256 MB PC-100 non-ECC Dimm
Viper 550 AGP video w/ 16 Meg

Kernel 2.2.13 + valinux nfs patches

I'm quite sure there are no hardware problems with this box. It's been
utterly stable otherwise.
