If you're using a pen and paper, it gets a lot easier if you preprint an
'oops form' with all the headers, into which you just write the numbers -
there's less to write and you're far less likely to miss bits out.
We probably ought to throw together such a template for each architecture and
stick it in the Documentation directory for such purposes.
