[PATCH] Journalled quota

Hi Andrew!
I'm sending you a new version of journalled quota patch. I'm sorry it
took so long but there was a deadlock which was hard to catch and I
needed to study for an exam from a group theory so...
I documented the locking so hopefully the code is easier to read now.
If you still think there is missing some info or you would like to see
some other changes feel free to tell me.

PS: I have also created patch with separated locking changes which are
valid for vanilla kernel (should fix some potential deadlocks). After
Chris Mason gives it also some testing I submit them for inclusion in
vanilla kernel.
