[PATCH 2.4.27] Forcedeth v28 backport

This patch backports version 28 of the forcedeth network driver to the 2.4 kernels.
It is against 2.4.27.

These two files are patched:
linux-2.4.27/drivers/net/forcedeth.c
linux-2.4.27/include/linux/pci_ids.h

The v28 forcedeth.c is taken from 2.6.8-rc3 with these minor changes:
- all occurrences of msleep changed to mdelay
- invocations of synchronize_irq changed to take no parameters
- SET_NETDEV_DEV calls removed
- module_param call changed to MODULE_PARM
