[Intel IOMMU][patch 3/8] Generic hardware support for Intel IOMMU.

Actual intel IOMMU driver. Hardware spec can be found at:
http://www.intel.com/technology/virtualization

The current version of spec is slightly outdated, but an updated version of
spec is in works and comming very soon. I will send a note to the list
when new updated spec is available. For the most part existing spec should be
good for basic understanding of the mechanics of the IOMMU.

This driver sets X86_64 'dma_ops', so hook into standard DMA APIs. In this way,
PCI driver will get virtual DMA address. This change is transparent to PCI
drivers.
