[PATCH] ide-cd: remove struct atapi_capabilities_page (take 2)

* Remove struct atapi_capabilities_page.
* Add ATAPI_CAPABILITIES_PAGE[_PAD]_SIZE define.

* The buf[] array access indexes were swapped between 'curspeed'/'maxspeed'.
