applied, thanks Yinghai. Again, very nice cleanups.
I guess we could remove mach-numaq real soon?
The APIC/IPI stuff it uses seems quite close to standard, with a few
quirks.
SMP bootup details seem a bit special in mach_wakecpu.h.
