[RFC v2][PATCH 33/35] asm-generic/ptemap.h for HIGHPTE implementations

x86 and frv have identical CONFIG_HIGHPTE implementations
of the pte mapping functions. Consolidate and move these
to asm-generic/ptemap.h.
