[PATCH 0/6] cleanups to lazy FPU restore code

These go on top of Oleg's patches.

The mechanism to disable lazy FPU restore is inscrutible
in several places, and dubious at best in one.

These patches make things explicit.

They also get rid of some apparently useless or redundant
code, and make fpu_lazy_restore work when in eager FPU mode.
