[PATCH v2 03/72] ncr5380: Eliminate PDEBUG*, TDEBUG* and DTCDEBUG* macros

Replace {P,T,DTC}DEBUG_INIT with NDEBUG_INIT. Remove dead debugging
code, including code that's conditional upon *DEBUG_TRANSFER.
