[PATCH v4 0/2] Update SCSI target removal path

This is a follow up to "scsi: Add intermediate STARGET_REMOVE state to
scsi_target_state".

Changes to v3:
* Incorporate James' suggestions for the commit messages.
* Re-add accidently dropped Reviewed-by tags.
