Can you try with

+	{"TPA6130A2 HPLEFT", NULL, "LLOUT"},
+	{"TPA6130A2 HPRIGHT", NULL, "RLOUT"}

being replaced with

+       {"TPA6130A2 LEFTIN", NULL, "LLOUT"},
+       {"TPA6130A2 RIGHTIN", NULL, "RLOUT"},
