[PATCH v2 0/3] kdb: Fix locking in vkdb_printf()

This patchset fixes some races that I noticed when looked
over the kdbd_printf() code. v2 is based on the feedback
from Daniel Thompson and Sergey Senozhatsky.

Changes agaist v1:

  + Completely remove kdb_event (Daniel)
  + Avoid using smp_processor_id() (Sergey)
  + Better describe the whole situation in 3rd patch (Daniel)
