[PATCH -next] lightnvm: fix possible memory leak in nvme_nvm_get_l2p_tbl()

'entries' is malloced in nvme_nvm_get_l2p_tbl() and should be freed
before leaving from the error handling cases, otherwise it will cause
memory leak.
