[PATCHv4 03/12] arm/mm: Provide pmdp_establish() helper

[Kirill A. Shutemov]

ARM LPAE doesn't have hardware dirty/accessed bits.

generic_pmdp_establish() is the right implementation of pmdp_establish
for this case.
