[PATCH v6 23/28] x86_64/asm: change all ENTRY+END to SYM_CODE_*

Here, we change all assembly code which is marked using END (and not
ENDPROC). We switch all these to appropriate new markings SYM_CODE_START
and SYM_CODE_END.
