[PATCH] libnvdimm, pmem: Fix badblocks population for 'raw' namespaces

The driver is only initializing bb_res in the devm_memremap_pages()
paths, but the raw namespace case is passing an uninitialized bb_res to
nvdimm_badblocks_populate().
