[PATCH 1/3] Use a struct for the on-stack 'struct iovec' cache for user sg requests.

rw_copy_check_uvector() has an optimisation to save the kmalloc() for
small SG arrays.
However nothing really guarantees that the actual size of the (on stack)
cache actually matches that passed to the function.
Since all the callers that pass a cache use the same size (UIO_FASTIOV)
embed the array inside a struct so that the compiler checks the size.
This also saves the size being passed through several layers.
