I think you can make it much more readable by introducing few variables:

virt = (void *)kbuffer + i * HV_HYPPAGE_SIZE;
phys = slow_virt_to_phys(virt);
pfn_array[pfn] = phys >> HV_HYP_PAGE_SHIFT;
