I found that ptg_page_shift and ptg_pmd_shift need
EXPORT_SYMBOL_GPL for cases where code compiled
as a module is using PAGE_SIZE/PAGE_SHIFT or
PMD_SIZE/PMD_SHIFT. Some of the others below
might also need EXPORT_SYMBOL_GPL.
