[PATCH v3 09/11] mm, swap: add debugfs counters for vswap

Add /sys/kernel/debug/vswap/ with two counters:

* used: virtual swap slots (pages) currently allocated
* alloc_reject: cumulative pages that failed to get a vswap slot
