I had this problem under 2.1.48 today, whilst copying files between
partitions.
It appeared first on the new partition, /dev/hda2.
Also, the directory usr/ disappeared from the root of /dev/hda10, although
there are no errors for that partition in my log file (see below).
Unmounting and re-mounting cured the problem - usr/ reappeared.
