I remember reading about this example:
...even 99.99% accuracy is not enough... if 1 out of every 10,000 W's are
changed to S's, you'll have few problems with a Mr Woodword's name being
changed to Mr Soodword, but you could have a lawsuit on your hand if the W
in Mr. Whitman's name is changed to an S.
