A combination of the two could work beautifully:

- Permissions are set in /etc/devfsd.perms or whatever. The file is of
course in human readable format and used by devfsd on startup and on
receiving a SIGHUP.
- Devfsd can itself modify this file to reflect any chmod'ing or chown'ing
in /dev. This is done automatically at once when devfsd notices a change
in permissions in /dev.

Can anyone tell me whats wrong with this solution apart from having to run
devfsd to save file modes and the problem you have when opening the file
in an editor, doing a chmod, and then saving the file?

As far as I can see it could work beautifully, but there might be some
points that I have overlooked.
