'D' processs

While trying to 'swapon /dev/fd0' (so I know when the kernel is about to
swap itself to death), swapon got stuck in the 'D' state with this in the
logs:

ll_rw_block: device 02:00: only 1024-char blocks implemented (4096)

I had run 'mkswap -c /dev/fd0' previously.

100 0 10771 1 0 0 824 272 end D ? 0:00 swapon /dev/fd0

Fortunately, I was able to reclaim the console with some selective killing
although the process is still hanging around.
